Where can I find a description of OPL3's anatomy and functions --
websites, books, etc.?

In particular I am looking for information on how the digital FM codes
generate sound, which codes generate which sounds, and how those codes
for the central channel of each instrument differ from the codes for
the seperate left and right channels of those instruments.
