Anyone know anything about a bootleg, Freak Out? Outside of where it
was manufactured and the date, I have no background on it.

Recorded at the Toronto Rock 'n' Roll Revival in September 1969.

The bootlegs have surfaced under different titles including "Freak Out".
"Freak Out" has been used for basically the same bootleg but with some minor
changes like the cover, spelling, names of the song.

Possibly the titles you will see are:
Painting a Picture is really No Longer Umpire
Instrumental is really Lay Down and Die, Goodbye
I've Written Home to Mother is really Lay Down and Die, Goodbye
Freak Out Song is really Don't Blow Your Mind
Nobody Likes Me is Nobody Likes Me
Science Fiction is really Fields of Regrets

"Going To The River" and "Ain't It Just Like A Woman" were not performed by
Alice Cooper (possibly by Ronnie Hawkins).

Is this the kind of info you were looking for?
