Question 1: What do I need to do to get a vanilla PC VGA monitor to work
from a sub-D15 male port marked D/V Port, where the manual says "connect
to a VGA converter"?
I would go ahead and just try a gender changer (or a cable with the
appropriate gender on each end. It would probably work, and the
risk of damaging anything is very low.

Question 2: Anyone have recommendations for a TV Tuner card that will run
under Linux?
That is more of a Linux question than a video question.

Question 3: Can I connect the threaded-connector coax output of an RF
modulator into the BNC-connector coax video input of the DVR?
Unlikely. From your own narrative you understand that RF modulated
video is NOT the same as baseband video. It took a modulator to get
baseband video (from an RCA or BNC connector) into RF (a TV
channel on a threaded "F-connector")
